Provider Manual
Welcome to the 2009 Provider Manual combined for Professional and Institutional Providers.
This edition of the 2009 Provider Manual provides an overview of Company products, policies, procedures and programs for both Professional and Institutional Providers and replaces any earlier versions of the Professional Provider Manual and the Hospital Reference Manual.
Two versions of the Provider Manual are offered below.
The Web Optimized version is structured in a convenient new format to enhance its viewing on the Web, including a Table of Contents divided into informational segments to assist in easily finding the information important to you. The Web version allows you to move quickly within and between sections of the Manual, as well as to specified internet sources via active links (designated by a colored outline around text). Additionally, there is a navigation bar at the bottom of the viewing page, allowing you to page forward, backward, return to the Table of Contents or view the entire page in the Full Screen mode.
The Print Friendly version permits those wishing a paper version to print the entire Manual or a portion of it without having to navigate through Sections to individually print them. Select “print” and specify whether you want to print “all” or identify the page range that you wish to print.
